install a new SMS Secondary Site. However, the Secondary will be in a
completely different domain to it’s parent Primary site so do I need to
establish a two-way trust between the Domains to get this to work?

You don't have to have trusts between two domains to set up a parent child
relationship. You can use pass through authentication with local accounts.
If you install the secondary site from the primary site, then the primary
site's SMS service account must be a local admin on the secondary site
server, in order to access the root of the NTFS drive to copy down the
bootstrap service and package file.
If you install the secondary site locally from the SMS CD, you only need an
address on the secondary site that has RWCD rights to the parent site's
SMS\Inboxes\Despoolr.box\Receive folder. This account is designated during
the secondary site installation in the install wizard (you must manually
create the address account and manually grant it the permissions required).
Then you'd create an address account from the primary site to the secondary
site with the same rights. What I do in my training classes is to create the
account locally at each site, and in the address to the remote site, I
specify the remotedomain\account to be used.
